If you are a finalist and have been offered an optional reassessment, you must use this form to confirm your decision. You have the following options:

1. To undertake ALL of the optional reassessments offered to you at the next available opportunity, as a resit without residence.
2. To undertake SOME of the optional reassessments offered to you at the next available opportunity as a resit without residence.
3. To NOT undertake optional reassessments and to receive your stated award, thereby forfeiting your right to resit any modules you have failed.

This form will open for responses following the release of results. The deadline is Wednesday 15th July 2026. If you do not complete this form by the deadline, you will not be registered for any reassessments and will graduate as expected.

The data recorded on this form will only be used for the purpose of notifying your academic department of your choice and the central administrative teams responsible for managing the delayed reassessment process. You will receive an email confirmation of your submission.
